Manufacturer recommended test timing based on therapy and dose:

One (1) 6.0 mL Whole Blood in a Clear Red Top Tube - No Additive (additional discard tube), FOR LINE DRAWS ONLY
One (1) 3.0 mL Whole Blood in a Clear Red Top Tube - No Additive (discard tube), ALL DRAWS
Two (2) 2.0 mL Whole Blood Dark Blue Top Tubes - 9NC Coagulation Sodium Citrate 3.2%
(Specimens may be collected from indwelling catheter (arterial or venous) if 21G or larger bore needle is used and at least 5.0 mL whole blood is discarded PRIOR to collection of kit. Note: first tube of kit is a no additive 6.0 mL tube. This means that drawing from a line would require a 5.0 mL discard + all three tubes in the collection kit.)

1. Read all instructions before collecting samples. Collect specimens using 21G needle only. Do not uncap tubes. Collect additional blood tests after this kit is drawn.
2. Collect all tubes in number order 1 - 3. Blue top tubes are 2 mL draw- fill to black line (1/2 full). Do not underfill.
3. Gently invert tubes 5 times. Do not create bubbles.
4. Label tubes with patient name, DOB, date and time of collection, and collector's employee ID. Place 'DO NOT SPIN' labels around caps of tubes after filling. Return kit tubes to original collection kit bag. Send any additional tubes collected separately. Do not refrigerate.
5. Hand deliver STAT to lab. Do not send via pneumatic tube system (CTS).

The VerifyNow P2Y12 Platelet Reactivity Test is intended to measure the level of platelet P2Y12 receptor blockage resulting from P2Y12 receptor blocker drugs, such as clopidogrel and prasugrel. The PRU reference range in P2Y12 receptor blocker drug-naive population is 182-335, according to manufacturer's package insert. In general, PRU values lower than 180 are associated with expected antiplatelet effect. Factors that contribute to an inadequate response are drug interactions, underlying health conditions, non-compliance, and inherent platelet disorders. Drug response is also dependent on dosage and timing of testing. Patients taking P2Y12 inhibitors are at increased risk of perioperative bleeding due to platelet dysfunction from drug effect. It is recommended to discontinue use of P2Y12 drugs for 5-7 days prior to surgery to allow restoration of platelet function. PRU values greater than or equal to 180 are indication of no drug effect. This assay does not test aspirin drug response or platelet dysfunction unrelated to P2Y12 receptor blockage. Clinical decisions cannot be based solely upon this test result.
