Patient Preparation:  
Specimen Requirements: 2.0 mL Serum from a SST Gold Top Tube
Collection Instructions:  Specimen should be drawn by venipuncture.  Do not collect specimen from a central line.
Minimum Volume: 1.0 mL Serum

Test Comments:  Although a negative Platelet Factor 4 (PF4) result (OD <0.399) does not totally exclude a diagnosis of Heparin Induced Thrombocytopenia (HIT), this test is about 95% sensitive for detecting antibodies to the PF4-Heparin complex.  No treatment decisions should be made based solely on this lab result, but should be made in conjunction with other laboratory findings and correlation with the patient's clinical condition.
 
Not uncommonly, heparin therapy induces Platelet Factor 4 (PF4) antibody without clinical heparin induced thrombocytopenia (HIT).  A positive PF4 result does not confirm a diagnosis of HIT.  While a strong positive value (OD >1.000) increases the likelihood of HIT, a functional assay, such as the Serotonin Release Assay (SRA) is more specific and is recommended to assist with the diagnosis, if clinically warranted.  No treatment decisions should be made based solely on this lab result, but should be made in conjunction with other laboratory findings and correlation with the patient's clinical condition.
