Specimen Requirements: 1.0 mL Serum from a Red Top Tube in a Plastic Vial
Collection Instructions: Specimen should be drawn immediately prior to next dose
Minimum Volume: 0.2 mL Serum
Neonatal Volume: 0.3 mL Whole Blood Red Microtainer (No Gel)
Transport & Storage: Temperature/Stability: 7 days Refrigerated

Rejection Criteria: Do not collect in a Serum Separator Tube (SST or PST)
Reference Range: 5 - 20 ug/ml
Critical Ranges: >27 ug/ml
Test Comments: The goal trough concentration for patients with endocarditis, meningitis, osteomyelitis, or hospital-acquired pneumonia will be 15-20 UG/ML.  For pathogens with Vancomycin MIC of 1 UG/ML, trough concentrations must be at least 15 UG/ML to achieve adequate AUC:MIC
