Specimen Collection & Preparation

Specimen Requirements:

1.0 mL Serum from a Red Top Tube in a Plastic Vial
Minimum Volume:
0.25 mL Serum*
Neonatal Volume:
0.3 mL Whole Blood Red Microtainer (No Gel)*

* This volume does not allow for repeat testing

Collection Instructions:

Specimen should be drawn immediately prior to next dose

Gel Barrier Tube (SST or PST)


Transport and Storage:
  • Ambient (18-24°C): 48 Hours
  • Refrigerated (2-8°C): 14 Days
  • Frozen (-20° C or colder): 12 Months

Clinical Interpretation

Reference Range:

5 - 20 ug/mL


Critical Ranges:

> 27 ug/mL


Test Comments:

The goal trough concentration for patients with endocarditis, meningitis, osteomyelitis, or hospital-acquired pneumonia will be 15 - 20 ug/mL.  For pathogens with Vancomycin MIC of 1 ug/mL, trough concentrations must be at least 15 ug/mL to achieve adequate AUC:MIC.
