Specimen Requirements: Tissue/Object removed from patient in 10% formalin
Collection Instructions: All surgical specimens must be labeled with the Patient's Name, Idendification Number,Physician, and source of specimen

Tissue/Object, Site/Source must be written on the container and requisition.
Transport & Storage: Temperature/Stability: Ambient
Reference Range: Refer to Interpretive Pathology Report, Level testing and/or combination Level testing completed will be determined by source and diagnosis
Test Comments: If additional reflex testing is needed turn around time will be delayed
